Advance Care Planning

Advance care planning is the process of planning for a person’s future health and personal care. Advance care planning helps ensure that an individual’s choices are respected for future medical treatment.  During this process, their beliefs, values and preferences are made known, in order to guide future care in the event that the person is unable to make decisions or communicate.

There are two main aspects to advance care planning:

A range of documents may be used to express the individual’s values and preferences for care and treatment. These may include:

Advance Care Planning Support Service

A Registered Nurse who has undertaken specific training in Advance Care Planning is available via appointment to visit you and or family members at home or you can attend the community health centre, The nurse will provide information, resources and guidance on the process to establish an Advance Care Plan.

Our nurses will conduct a comprehensive assessment, generally in your home, and will work with you to meet your needs in the best way for you. The team can also conduct community group education sessions on a range of health related topics on request. NB GLCH delivers this service to residents of a catchment area roughly contained by the Tambo River to the west, the Bruthen-Buchan Road to the north and Hospital Creek to the east. If you live outside this area your care will be provided by another service provider.
