Palliative Care : Provides specialist health care and practical support

The Community Palliative Care service provides specialist health care and practical support aimed at enhancing the quality of life of people who have advanced disease or a life limiting illness.

The palliative care team works closely with you, your doctor and your family to help you implement your preferred care options in the way that suits you best.

We provide:

NB GLCH delivers this service to residents of a catchment area roughly contained by the Tambo River to the west, the Bruthen-Buchan Road to the north and Hospital Creek to the east. If you live outside this area your care will be provided by another service provider.

Palliative Care Nurse Practitioner

Our nurse practitioner provides specialist assessment and advice in collaboration with your GP, local nursing services and visiting palliative care physicians as required.

GLCH delivers this service throughout the East Gippsland Shire.

Our nurses will conduct a comprehensive assessment, generally in your home, and will work with you to meet your needs in the best way for you. The team can also conduct community group education sessions on a range of health related topics on request.
