Home Care Packages : Staying in your own home
Your local My Aged Care provider, servicing all of East Gippsland
A Home Care Package is a flexible package of care services tailored to meet the individual needs of people who would like to remain living in their own home. The package is coordinated by a care manager at Gippsland Lakes Complete Health and is funded by the Commonwealth Department of Health.
Our care managers work in partnership with you and your family to plan your package of care and to develop an individual budget to support your level of care. You can choose what level of input you would like from your care manager.
Your package of care may include assistance with personal care such as showering, domestic assistance such as housework and shopping, or possibly help to access social activities. Other types of assistance that may be provided include:
- Meal preparation
- Laundry
- Transport
- Garden Maintenance
- In home respite
- Nursing care
- Allied health such as podiatry, occupational therapy, physiotherapy
- Continence
- Access to specialist appointments
- Equipment
To be eligible to receive a Home Care Package you must be assessed by the Aged Care Assessment Service (ACAS). This assessment can be organised via the My Aged Care website www.myagedcare.gov.au or by phoning the National Call Centre on 1800 200 422. From February 2017 Home Care Packages are fully portable between approved providers.
