This service provides assessment, support and education in the management of Parkinson’s Disease.
Parkinson's Nurse
The Parkinson’s Nurse role is here to support clients and family in their management of Parkinson’s Disease. The nurse works closely with medical specialists, GPs and other healthcare professionals like speech pathologists, occupational therapists and physiotherapists.
The Parkinson’s Nurse can provide a range of services including:
- Understanding on ‘What is Parkinson’s’
- Monitoring of symptoms
- Information on how best to manage your symptoms
- Referrals to other healthcare professionals as required
- Care coordination.
The Parkinson’s nurse can arrange a home visit with you and your family or carers, assess your current needs and develop a plan with you and how best to support your current needs.
